Many impediments have been identified in the collaboration between child and adult systems and little is understood about which programs tend to collaborate well across the "age divide" and what factors might encourage programs to do more when facilitating child-to-adult service transitions. This webinar will present findings from a study of programs in transition-age youth systems of care in communities working to improve these systems and will include a discussion of implications for improving systems of care for transition-age youth.
