Core Element Overview
A Family Partner works with a caregiver and service providers to identify their shared goals for the services the family is receiving. They meet with the caregiver to understand their priorities and encourage their participation in developing service and treatment plans. The Family Partner participates in service and treatment planning meetings to ensure that the caregiver’s voice is heard. Throughout the goal planning process, it is important to celebrate all family achievements.
