Core Element Overview
Transition planning means preparing for service and life changes and navigating through them so that the family, service providers, and formal and informal networks are all supporting the youth and family during the process. The Family Partner plays an important role in helping the caregiver handle the stress and uncertainty that can come with periods of change. The Family Partner will work with the caregiver and other providers to create a transition support plan. The Family Partner also helps the caregiver gain the specific skills and access the services they will need to be successful in the new situation. If the caregiver decides to stop participating in services, the Family Partner should reach out to understand the problem and find a solution if possible.
